This Lightening Talk will describe a federally funded, nurse-led mobile health program that addresses the health consequences of climate change and the social determinants of health (SDOH) disproportionately impacting low socioeconomic populations and communities of color. Climate change is an urgent threat to public health and health equity that health professionals must have the knowledge to address. Participants will learn about our pre-clinical workshop that equips students with strategies to manage the intersection of inequitable environmental exposures, climate change, and SDOH through interprofessional collaboration. The program facilitates student-led home visits and community health education in urban areas, focusing on climate mitigation strategies for extreme heat, poor air quality, emergency preparedness, and vector-borne diseases. Students also complete health assessments and screenings and initiate referrals to primary care and mental health services as needed. Nursing students work alongside faculty and graduate students in occupational therapy, physician assistant, and audiology, leading to enhanced interprofessional practice skills. The presentation will provide climate-related resources and assessment tools to integrate into healthcare practice. The program has reached ten sites across seven communities, conducting 70 home visits with 32 clients. Feedback indicates that students gain a comprehensive understanding of the multifaceted impact of environmental, social, structural, and economic factors on health. Community constituents value the student-led sessions as they address critical issues impacting their health that are often not tailored to their needs.
