Building Community-Centered Health Initiatives: A Roadmap for Faculty to Develop Interprofessional Education Projects

The UCI Interprofessional Education & Practice (IPEP) Collaborative, committed to transforming health professions education and healthcare delivery, organized a free community health fair aimed at advancing interprofessional care through practice-education partnerships.
Held at Orange County's oldest African American congregation, the health fair engaged over 200 residents. This event was unique in its inclusion of multiple facets demonstrating true interprofessional practice, including collaborations between nursing, medicine, pharmacy, public health, early education outreach programs, a pediatric mobile eye clinic, and health and wellness coaches. Even more unique was the use of small interprofessional student groups providing key healthcare screenings and education. Students engaged in direct patient care experiences with peers from other professions, witnessing firsthand the benefits of interdisciplinary teamwork and demonstrating what interprofessional teams look like to the community. This event highlights how collaborative efforts can bridge the gap between education and community health needs.

This Lightning Talk will describe the organization and outcomes of a free community health fair, demonstrating the power of interprofessional collaboration to improve health outcomes and educational experiences, as well as provide attendees with practical insights into developing and implementing community health initiatives that enhance interprofessional education and practice. Participants will learn about the planning and execution processes, highlighting the importance of faculty and student involvement in community outreach. The discussion will cover measurable learning outcomes, improved health equity, and the impact of interprofessional teams on health Learners will gain knowledge on maximizing student contributions in practice settings, facilitating health and learning outcomes through patient education, outreach, and screening. The talk will also showcase how integrating social determinants of health into community health projects can improve population health and provide better care.

By fostering community-centered partnerships and collaborative learning, the UCI IPEP Collaborative exemplifies how educational institutions can play a pivotal role in community health. The health fair model demonstrates effective strategies for relationship-building across education and community settings, leveraging interprofessional practice to improve health outcomes.

This presentation will serve as a model for other institutions, offering a roadmap for developing interprofessional education projects that make a tangible impact on community health. By addressing both clinical and social determinants of health, the UCI IPEP Collaborative sets a benchmark for community-centered, interprofessional healthcare education and practice, contributing to better care, better value, and better education in practice and community settings.

Accreditation Details

In support of improving patient care, this activity is planned and implemented by The National Center for Interprofessional Practice and Education Office of Interprofessional Continuing Professional Development (National Center OICPD). The National Center OICPD is accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

As a Jointly Accredited Provider, the National Center is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. The National Center maintains responsibility for this course. Social workers completing this course receive continuing education credits.

The National Center OICPD (JA#: 4008105) is approved by the Board of Certification, Inc. to provide continuing education to Athletic Trainers (ATs).
