The Community Health Mentor Program in Arizona is a collaboration between the three public universities: Northern Arizona University, University of Arizona, and Arizona State University. In this program students from PT, OT, PA and MD programs work together to learn from the lived health experiences of members of the community who have either chronic medical conditions and/or disabilities.

Students from all healthcare education programs must graduate prepared for collaborative practice. It can be a challenge to meet the interprofessional educational needs of hybrid learners. This lightening talk will describe how this program successfully expanded to accommodate rural learners and hybrid learners from the new PT and OT hybrid educational programs. The talk will include a patient-mentor from our program describing what his experience is like in the program as he has participated prior to the incorporation of hybrid learners and, now this year, works with both a traditional team and a hybrid team.

This talk will include 1) description of our program, 2) description of how this program was adapted to incorporate hybrid/rural learners, 3) the experience of participating in the new hybrid teams from a patient-mentor co-presenter.
