Patients Partnering with Health and Education Systems to Drive Positive Change

Since 2016, the Nexus Summit has been co-created with patients, family and community members. In 2023, our Patient Responders shared the personal experiences that drive their passion for change that positively impacts patient care and outcomes. In 2024, we continue to work together with our Patient Advisors and Patient Responders to advance the patient engagement conversation with concrete strategies and positive examples of patients and family members actively partnering with health and education systems in governance, policy and workflow decisions that shape their healthcare experiences.

 

The United States healthcare system does not always achieve the goal of effectively serving the patient, family, community, or the provider and their team in the delivery of care. But systems change is possible if we create meaningful partnerships and focus on Quadruple Aim outcomes that drive health equity. During this plenary, a panel of academic health system leaders, clinician educators, and patient advocates will have a conversation about practical steps to engage patients, families and communities meaningfully at the Nexus of practice and education to drive positive change. This panel will challenge each of us to consider how to take practical steps toward deepening meaningful patient and community engagement in our interprofessional work.

 

At the conclusion of this plenary, participants will be able to:

  • Recognize authentic patient/ provider or patient/ system partnerships that impact health system outcomes.
  • Identify one opportunity to support impactful patient engagement in their local practice or education setting
  • Describe how to support practice leaders and patients to engage in meaningful partnerships that drive health and/or learning outcomes that matter.
Accreditation Details

In support of improving patient care, this activity is planned and implemented by The National Center for Interprofessional Practice and Education Office of Interprofessional Continuing Professional Development (National Center OICPD). The National Center OICPD is accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

As a Jointly Accredited Provider, the National Center is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. The National Center maintains responsibility for this course. Social workers completing this course receive continuing education credits.

The National Center OICPD (JA#: 4008105) is approved by the Board of Certification, Inc. to provide continuing education to Athletic Trainers (ATs).

Physician Assistants: The American Academy of Physician Assistants (AAPA) accepts credit from organizations accredited by the ACCME.

Nurses: Participants will be awarded contact hours of credit for attendance at this workshop.

Nurse Practitioners: The American Academy of Nurse Practitioners Certification Program (AANPCP) accepts credit from organizations accredited by the ACCME and ANCC.

Pharmacists and Pharmacy Technicians: This activity is approved for contact hours.

Athletic Trainers: This program is eligible for Category A hours/CEUs. ATs should claim only those hours actually spent in the educational program.
