Fellowship Mix-Up Day: Interprofessional Collaboration, Merging of the Minds

A large, system-based, accredited transition-to-practice program for Advanced Practice Registered Nurses (APRNs) and Physician Assistants (PAs), known as the Advanced Practice Provider (APP) fellowship, currently operates six specialty tracks, including adult critical care, primary care, urgent care, pediatric critical care (PICU), neonatal critical care (NICU), and general pediatrics.

The APP Fellowship program leaders and faculty team emphasize the importance of teamwork and collaboration, which led to the inception of what is termed fellowship mix-up day. An 8-hour didactic day was curated to promote interprofessional collaboration amongst six specialty tracks, divided into two categories, adult, and pediatric based fellowships.

The interprofessional practice environment for learning was optimized using three simulation scenarios, integrating all specialty APP fellowship tracks following the same patient through his/her medical journey. For the pediatric fellowship track, the simulation journey began with a NICU discharge, followed by an outpatient general pediatrics visit, which led to a PICU admission. On the adult side, a pregnant female presents to urgent care, later follows-up with her primary care due to various acute symptoms, which require hospitalization, leading to a critical care admission. Simulation debrief was led by a senior fellow from each APP fellowship track, which bolstered leadership and fellow led engagement. To prepare the senior fellow, a near- peer debrief tool was provided beforehand. APP fellowship faculty were also present during the simulations and debrief to provide guidance and feedback when needed. A total of 20 fellows participated in the group simulation/debrief experience. With group simulation, one fellow completed the simulation, while all other fellows observed. This approach created rich discussion and highlighted the abundance and/or lack of resources in each clinical environment.

To further support interprofessional collaboration and education, a pharmacist and cardiac fellow were invited to share knowledge on their perspective areas of expertise, providing an opportunity to engage APPs with healthcare colleagues, promoting dialog and building relationships.

Using Kirkpatrick’s model of programmatic evaluation, a post event survey revealed level 1 outcomes with learner engagement data exceeding the target with an average score of 4.86 on a 5-point Likert scale and level 2 outcomes demonstrating a 30% increase over baseline with confidence of application.

By integrating a multi-specialty simulation experience and interprofessional collaboration with guest lectures, a better understanding of clinical practice in different settings can be formulated, leading to enhanced communication amongst healthcare colleagues.

In support of improving patient care, this activity is planned and implemented by The National Center for Interprofessional Practice and Education Office of Interprofessional Continuing Professional Development (National Center OICPD). The National Center OICPD is accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

As a Jointly Accredited Provider, the National Center is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. The National Center maintains responsibility for this course. Social workers completing this course receive continuing education credits.

The National Center OICPD (JA#: 4008105) is approved by the Board of Certification, Inc. to provide continuing education to Athletic Trainers (ATs).
