Tatyana Gurvich, PharmD, CGP, is an associate professor of clinical pharmacy (clinician-educator) in USC Mann and an adjunct assistant professor in family medicine at the University of California, Irvine (UCI), Medical Center. For more than a decade, she has served as a geriatric clinical pharmacologist/clinical preceptor at USC Mann/UCI Medical Center Senior Health Clinic, a collaborative geriatric primary care practice. She has been involved in Geriatric Workforce Enhancement Program–funded activities at USC Family Medicine and the UCI Department of Geriatrics. She participated in multiple programs which have been focused on IPE and outreach to primary care providers in the community.

The WHO estimates that Social Determinants of Health (SDOH) “account for 30-55% of health outcomes” and “can be more important than health care or lifestyle choices in influencing health” (2024). Team-based collaborative care that is person-centered is necessary for addressing the SDOH that lead to health inequities and disparities among vulnerable populations. Experiential education in community contexts prepares the interprofessional team to be collaborative practice ready when entering the workforce.
